Pervasive gaps in ED-to-community care transition interventions for older adults with cognitive impairment must be addressed.  This GEAR 2.0-ADC scoping review aimed to describe emergency department care transition interventions delivered to older adults with cognitive impairment, identify relevant patient-centered outcomes, and determine research areas for future investigation. Key gaps in ED-to-community care transition interventions were identified. These key gaps were combined with stakeholders’ assessments and prioritizations to help identify relevant patient-centered outcomes and clarify priority areas for future investigation to improve ED care for individuals with impaired cognition.
